The action box does not show up in QM01 (create)
You will only see the action box after creation by using QM02 (edit)

Would this be better posted on the Quality Management forum? You seem to be identifying quality notification types.
QN types are highly customizable so exactly how your firm is employing action boxes with the Q3 QN type may not be something the forum can address easily.
