This sounds like a custom Action Box to create the QD (I'm assuming QD is a custom Quality Notification Type).
So, you may need to check the configuration settings for Notification Type Q3 and see what action boxes are configured.
Sometimes people configure the action to only show up on the Change transaction (QM02) vs. Create (QM01), so you may want to check that as well.
