[sap-hr] Quota not accruing correctly for small group of employees

Urgent cry for help - payroll issues again this week.

Quota accrual has worked well for many months. Now we are having an issue where a small group of employees are not receiving any quota and another group are receiving double. In this personnel area we have 67 non-exempt employees.
Background info:
1. We are paid and accrue quotas biweekly in 24 periods.
2. Any month with three pay periods we only accrue quota first and third period.
3. We recently completed upgrade to EHP4.
4. Only 25 of the 67 employees are having the issue.
5. 12 EEs have achieved the level of 6 hours per pay period but received no annual leave accrual.
6. 13 EEs have achieved the level of 5 hours per pay period but have received 11 hours per pay period the last two periods.
7. All 25 employees in the issue group received the correct sick leave accrual which is 3.7 hours per pay period regardless of years of service. (not seniority based).
8. We have checked all time intervals in Table T559E and all settings there are correct.

Can anyone suggest other settings or areas where the issue might reside? Thanks in advance.
