A duplicate key insert often points to a number range overlap issue. Check your number ranges for example, if you number range goes from 10000 to 19999, when you reach 19999, it will go back to 10000.

Subject: SD BOM Dump - Open SQL array insert produces duplicate records in the database
While creating a Sales Order with reference to a BOM - pricing at the header level the Sales Order gets created and saved. However, when we display the sales order, a message appears 'SO does not exist or has been archived'.
A dump is seen in ST22 'Open SQL array insert produces duplicate records in the database'. This occurs because of duplicate entries getting created in table KONV (i.e. SD pricing table).
How do we resolve this issue, so that the dump does not occur. Please suggest.
