Since it is wrong on all of the forms, it sounds like maybe your custom forms do not include all of the applicable tax form fields or exit processing.
We are on ECC 6.0 and use Adobe forms delivered by SAP so we don't have the customization that you have, but we show the following HIRE Act tax form fields in T5UT3 for form HR_F_W2:
Fld No Start Date End Date Eval Dfn TaxFormGroup
EWGP 00 1/1/2010 12/31/9999 1 W2CC
EWGP 01 1/1/2010 12/31/9999 K Spec Form exit processing
EWGP 02 1/1/2010 12/31/9999 R
I don't have access to the form builder in our system since it is a development object so I can't see how the code is determined for sure, but none of the other Box 12 items that get codes assigned have the special form exit processing that this one does.
Maybe you should make a new copy of the newest SAP version of the form and redo your other customizations?

In an effort to resolve a W2 Box 12 Code issue, I am wondering. If through the IMG you go to Payroll, US, Tax Reporter, Tax Form groups, Defining Tax Form Groups for Non-tax wage types, Country 10... if you go to W2CC, what do you see for assigned wage types? Anything?
We are having a problem with the CC code not showing up on the W2 Box 12. The correct $ amount is there, but instead of CC and differing numeric code shows instead.
