Not sure malicious, but if the case, you might want to gain an understanding of who has access to SE16N auth object S_DEVELOP with ACTVT = 02. This plus placement of certain command in transaction bar would set "Maintain entries" flag as checked. Further to that, does anyone have debug replace access in your productive environment, etc. Tough one to really find at this point, sorry to not have more.
