three things:
1. when mat. determination master is created, the second last column is of
UoM, where u can enter the "cases"(customized) as UoM so that system
replaces it with original materials UoM. you can also select material
record and go to alternate material section and mention the UoM of other
material and see if it works
2. even if that is not maintained system picks up the UoM of original
material not substituted material
3. please check if in material master of alternate material in sales org 1
tab in delivery unit field is there any multiple maintained, may be system
is accepting values as per the value mentioned

Have you maintained any conversions(Like 2units=1 case etc...)
Alternatively you can check this in sales order also by going to
Item>>>sales A tab..
Just check when you got 2 units of ABC456 at sales order level & vice
versa..
