It depends on whether you are just upgrading with:
a)no functional change - Even here you need to assess existing custom program code, see what/how it is to be coded in the next upgrade level. There may not be any impact, but you still need to assess.
b)some functional change - Here the changes are treated as a mini-medium project.
You can find SAP doc's/notes re: the differences between the two versions.
There are 3rd party tools to assist in the upgrade analysis. Example: Intellicorp's LiveCompare
