Dear Deepak,
It is happen because while doing Goods issue user has entered order 2 times, so this way it is possible if you proper check through standard & exits.

I have not done this, our users have done this so that i dont know how to find this and how he has don the excess qty posting of GR in the transaction MIGO.Kindly provide me the solution.
