Standard answer: History is best kept in Notifications or for measurements, in Measurement Documents. You can produce a standard report or customize to bring these together.
Some create forms/checklists for better structure.
What are you doing now and what do you not like about it? Who enters the observations/readings and how (mobile? transactions?)
