Is this your only instance on the box? If not it could be that the files in /usr/sap/tmp are not owned by the user properly. so check the permissions on the files. It could also be that you have lost connectivity with the shared memory. Check for notes on saposcol shared memory, there is a command that can be issued at OS level that will assist on reconnect. Start with notes 1322232 and 548699.
