There could be 3 three things -
1) Have you updated the kernel at both the exe locations ie /sapmnt/SID/exe
and /usr/sap/SID/SYS/exe ???
2) If you Central instance and message services are running on a different host. Have you restarted them as well. ( If no then please do)
3) what is your OS and DB platform please make sure that disp+work , R3trans -v and tp -v all should show the new kernel level.
4) In dispatcher logs try to find out the keyword 'mismatch' if you find it solution in point 2) will definitely work.

We upgraded the SAP kernel patch 700 from 201 to 291,then start the instance, but the SAP MMC colour is red, it displayed.
