Or a major event like a terminal illness when you would want an action to stop the person being paid and their status to be set to inactive for head count reporting, maybe?

It is not advisable to use PA40 for minor events such as sick leave. In addition, unless you have enabled additional action in your system, SAP only allows one personnel action on any day. Is there any particular reasons that your customer wants to use PA40 for sick leave? If it is just for the purpose of calling up IT0014 screen, I will advise that you consider using dynamic action instead of personnel action. Hope this helps.
