That is what I would do. First get your list of employees who do have an active LTD plan (IT0168) by pernr.
Use an employee list or SE16/SE16N to identify all active employees eligible to enroll in LTD but exclude the pernrs of those who have an active IT0168.
The result will be those employees who don't have one but are eligible to enroll.

I would suggest using SE16N as opposed to Ad Hoc Query. It's a much easier reporting tool if you are only concerned with information contained in one specific infotype. If you can provide a list of all active employees by PERNR, you can run it against the LTD plan in question to find those that do not have it.
