It should be literally impossible. The key to the table is Client and User, so there is likely a difference between the "repeated" records in one of those two fields. Watch out for unprintable characters in the user field that look like spaces - check out the actual hex contents of that field. If that's your issue, then there's something wrong with how the users were created.
